  • Provides supervision of non skilled services.
  • Maintains confidentiality of patient and agency matters.
  • Prepares written instructions for care provided by home health aides and Certified Nursing Assistant.

» Home Health Aide/Certified Nursing Assistant 

  • Provides direct personal care to the patient in the home setting under direction of a registered nurse.
  • Utilizes safety measures in provision of care.
  • Maintains effective communication with supervisor regarding patient's condition.
  • Participates in in-service training as required.
  • Performs all personal care activities contained in a written assignment by a RN Case Manager of the home health agency.
  • Documents and submits accurate records of personal health care activities.
  • Observes appearance and gross behavioral changes in the patient and reports to supervisor.
  • Removes patient's medication container from the storage area; prepares necessary items, i.e. juice, water, cups, spoons, etc.; to assist the patient.
  • Reading and recording temperature, pulse and respiration.
  • Participates in case conferences.
  • Maintains confidentiality of all patient and agency matters.
  • Recognize emergencies and have knowledge of emergency procedures.
  • Demonstrates knowledge regarding basic infection control procedures.
  • Adheres to the role of the Home Health Aide in the home setting as defined in Florida statute for home health agencies.
  • Participates in training activities as requested annually.

  • Conduct care planning assessments to identify problems, eligibility for assistance, and need for services
  • Review financial, legal, or medical issues and offer referrals to geriatric specialists to avoid future problems and conserve assets
  • Screen, arrange, and monitor in-home help or other services
  • Escort to doctor's appointments and report back to family
  • Act as a liaison to families at a distance, making sure things are going well and alerting families to problems
  • Assist with moving to, or from, a retirement community, ALF, or nursing home

  • Set up filing system or update existing one
  • Providing financial statements and balance sheets
  • Organize and maintain records for banking and bill paying
  • Process medical claims from Medicare and private insurance agencies
  • Provide documentation to appropriate party under direction of client
  • Provide report for out of pocket medical expenses at year end
